MEET OUR STAFF
Chasity Hammond, RN, MSN, FNP-C
North Carolina born and raised, Chasity grew up in Charlotte, and now lives in Salisbury. She received her AD in Nursing from Cabarrus College of Health Science in 2008 and her BSN from UNC-Greensboro in 2016. Chasity's nursing experience was primarily in pediatrics and women's health. In 2019 she earned her Master of Science in Nursing from Frontier Nursing University and was certified as a Family Nurse Practitioner by the American Academy of Nurse Practitioners. She has been practicing in Palliative Care and Hospice since 2019. Chasity is a member of both the American Association of Nurse Practitioners and the NC Nurses Association. After seeing the disparities in healthcare firsthand, she dreamed of a way to advocate for patient rights and provide a higher standard of care. Rowan Health and Wellness is a product of Chasity's passion to provide compassionate, quality healthcare to her community while focusing on wellness and disease prevention. Chasity is a wife, mother, daughter, sister, friend, and most joyously a Mimi. She is also a lover of dogs, Disney and Panther’s football. When not taking care of patients, you can find her traveling, and spending time with her family.
Traci Nixon, RN, MSN, FNP-BC
In 2009, Traci earned her Master of Science in Nursing from the University of North Carolina at Charlotte and her Family Nurse Practitioner from the American Nurses Credentialing Center. With 13 years experience as a Registered Nurse in the Emergency Department, Traci realized the value of disease prevention. In 2010, she traded in the adrenaline of the ER for family practice and found a new passion in Primary Care. In 2021, Rowan Health and Wellness was born in hopes of providing care that was more consistent with her personal values while allowing for a better work-life balance. Traci enjoys taking time to get to know her patients and their families. She tailors her advice to fit the person she is caring for as much as the condition that is being prevented or treated. Traci has a solid reputation in her community as a Nurse Practitioner, parent and friend. Traci lives in China Grove with her husband, two kids and three dogs. She spends most of her time away from work volunteering and watching her kids play sports, but is always happy to travel, play or nap when given the opportunity.
Anna Gale, RN, DNP, FNP-BC
Anna Gale is a Family Nurse Practitioner with over nine years of nursing experience. She earned her Bachelor of Science Degree in Nursing from Messiah University in Mechanicsburg, Pennsylvania in 2012. After working for several years in cardiac acute care settings, she returned to Messiah University and completed her Doctor of Nursing Practice (Family Nurse Practitioner track) degree in 2020. Anna is a Board Certified Family Nurse Practitioner and licensed in North Carolina. She is an active member of the American Association for Nurse Practitioners (AANP) and Sigma Theta Tau, the International Honors Society of Nursing. Anna enjoys building relationships with her patients and their families. She recognizes the unique health journey of each person she meets and focuses on providing holistic, compassionate care. Anna resides in Salisbury, North Carolina with her husband, son, and rescue dog. She enjoys spending time with her family, cooking, and exploring the National Parks.
Patti grew up in Cabarrus County, NC. She received her Bachelor's degree in nursing from UNCC and her Master’s degree in nursing administration from UNC-Greensboro. She graduated from the University of South Carolina in 1992 and was certified as a Family Nurse Practitioner. She practiced for 11 years in developmental pediatrics before taking the position of coordinator of the nurse practitioner program at UNCC. While at UNCC she had numerous presentations and was awarded grants primarily in the area of bariatric medicine. Her work was published in Community/Public Health Nursing: Promoting the Health of Populations (5th ed). She spent the past 10 years practicing bariatric medicine.
Patti is married and has three grown children. In her spare time enjoys spending time with her family, especially her four grandsons. She also enjoys going to the beach and quilting.
